The Shire of Shark Bay is asking for your help with creating a new Health and Wellbeing Plan for our Shire. We are inviting our community to fill out an online survey, which takes about 15 minutes.
We want to hear your views about what good health and wellbeing means to you, and any difficulties you face in achieving that. Your feedback will help us identify community needs and areas for action so we can improve the health and wellbeing outcomes for all of the people who live, work and play in our Shire, both now and into the future.
Your privacy is important to us. To ensure survey confidentiality we do not ask for your personal identifying details and we will only be publishing aggregated data, not individual responses. Only the first six questions are compulsory, they tell us a bit about you, but do not identify you.
